CRESTVIEW ELEMENTARY REGISTRATION 2017-2018 SCHOOL YEAR

ALL STUDENTS – NEW & RETURNING

Online registration is open now…Go to student registration on Canyon ISD website.

RETURNING STUDENTS MUST BRING THE FOLLOWING:

  1. Proof of Residency (See below)
  2. Driver’s license of person registering student
  3. Any new immunizations

NEW STUDENTS MUST BRING THE FOLLOWING:

  1. Social Security Card
  2. Birth Certificate
  3. Immunization Record
  4. Driver’s license of person registering student
  5. Last Report Card
  6. Proof of Residency (must have current physical address)
    Acceptable:
    • Excel Energy statement
    • Atmos Energy statement
    • City Water statement
    • BFI Sanitation statement
    • Rent receipt on company letterhead
    • Notarized residency form
    • Direct TV, cable TV statement

STUDENTS WILL NOT BE CONSIDERED ENROLLED UNTIL ALL THE ABOVE ENROLLMENT REQUIREMENTS ARE MET.

IF STUDENT DOES NOT LIVE IN CRESTVIEW ATTENDANCE AREA, THEY MAY NOT REGISTER AT CRESTVIEW WITHOUT AN APPROVED IN-DISTRICT TRANSFER. THEY MUST REGISTER AT THEIR HOME SCHOOL.
